A gloriously ordinary life

You said you want to be:

  • supported to live independent lives in a way that meets the outcomes and goals you have set for yourself

  • able to live a gloriously ordinary life whilst still reaching for the bold aspirations that matter to you

As Social Care Practitioners we will:

  • value you, and your views, experience and knowledge
  • not make assumptions about you
  • be curious about you
  • take time to understand you, your aspirations and dreams
  • ask you the right questions
  • recognise that you may have bad days and set the bar at a fair height
  • see you as a person, not as your needs
  • provide the right advice and support at the right time, enabling you and your support network to find solutions in a way that works for you and your accessibility needs
  • when information, care and support is required we provide the right care, in the right place, at the right time
  • ensure you can be in the life you want to lead (self-care/resources/wellbeing)
  • work together with you to coproduce a plan for your care and support

As commissioners we will:

  • continually review and develop the quality of care and support services, working with people to determine what ‘good’ looks like
  • develop resources to proactively support people
  • develop links across communities that support you to connect with real people through real relationships in everyday life
  • see beyond your basic needs to understand who you are as a person
  • develop services that deliver support in a range of creative ways that are personalised to you
  • develop support services that see you as a person, not as your needs

To monitor how we are doing we will:

  • use quality checkers to independently review the quality of our services
  • undertake audits of quality assessment and support planning, and use case studies in line with our vision and priorities
  • continually learn and reflect through feedback and engagement with local people
